Bill for Replacement Costs Appeal Form

Please be advised that the library is using a standard replacement cost for lost items. The cost listed on the bills may be higher or lower than the actual replacement costs. If you believe the actual cost to replace an item is lower than the amount billed, we encourage you to appeal the amount billed. The appeal must be in writing using this web form or a form provided at the circulation desk.

IMPORTANT: If you request an appeal, we will make every effort to determine the actual replacement cost of the item. Instead of the standard $140, you will be responsible for the actual replacement cost, whether that cost is lower or higher, plus the processing fee. It is likely an appeal will result in a lower cost. However, an appeal could result in a higher cost.
